Transition of Care Services

ComForcare Home Care offers award-winning transition of care services for clients who are ready to be discharged from the hospital.  Patients can be discharged sooner than expected, so if this happens, ComForcare is available to help.  The “Hospital to Home” care program provides a caregiver to pick up a client from the hospital and receive all discharge information.  The caregiver will also be able to stop at the pharmacy for medications and medical supplies or the grocery store for food and personal supplies.  Once home, the home care provider will prepare meals, help the client get comfortably settled, and phone family members to provide an update and relay necessary care information.

Ongoing Home Care Support

Once a client is settled at home, home care responsibilities can be transitioned back to the family or ComForcare can continue providing part- or full-time home care services through the recovery period.  Home care services are available for individuals who are dealing with issues of chronic illness, post-surgery recovery or a serious injury.  Services are also available for new mothers.

Home care providers from ComForcare can also assist clients who are staying in other facilities such as a nursing home, rehabilitation center, and assisted-living community.  Services are set up on a personal basis, depending on the needs of each client and their family, so the highest quality of care is continually delivered.
